Self-Check

The self-check tool allows you to conduct a quick check of your symptoms and conditions, and provides you guidance of possible steps to take. This system is not intended for the diagnosis or treatment of disease or other conditions, including COVID-19. People with COVID-19 have had a wide range of symptoms reported, ranging from mild symptoms to severe illness. Symptoms may appear as many as 2-14 days after exposure to the virus.
This system is intended only for adults who are 18 years and older. If you are a staff member of NMSU, please check with your supervisor to determine whether running the self-check is a required condition before coming to work — in which case, you should answer positively to the final question about registering your self-checking activity.

DISCLAIMER: This self-check tool does not process, transmit, collect or retain otherwise any information unless you explicitly provide your consent for doing so at the end of the survey. If you give consent for this self-check tool to record the occurrence of your self-checking, the only information that this self-checking tool retains will be your email address, the date of the self-checking, and a YES/NO notation, depending on whether your self-check tool input indicates that symptoms have been detected.

Self-Report

The self-reporting tool allows you to voluntarily notify the NMSU Aggie Health and Wellness Center of a positive result from a COVID-19 diagnostic test. This notification will enable the center to conduct contact tracing and meet applicable State of New Mexico requirements and guidelines.

Self-Registration

The self-registration tool allows you to notify the NMSU Aggie Health and Wellness Center of locations you have visited. This information is useful to allow NMSU to conduct contact tracing in case of a COVID-19 positive case. The information will be maintained by NMSU Aggie Health and Wellness Center for a period of two weeks and discarded afterward. The information will be used exclusively by the NMSU contact tracing team for the purpose of detecting possible contacts and providing information to them to protect their health. undefined
